When drawing (Pen and paper), I do the basic sketch then outline.
When doing pixels, I have the habit of wanting to outline everything. Example, outlining the head, body, arms, etc. This leaves little to no room for adding detail.
I like to work with smaller pixels, 32x32 to 64x64. I can get a basic outline going, but when adding detail everything becomes a blurry mess.
I know this varies from person to person, but is it easier to start the sprite with a basic outline then work in detail. Or start straight from a final form.

The steps you take are up to you and what you feel will yield the results you're looking for. You've found the problem with your approach, so you ought to try something different. One suggestion could be perhaps using broader shapes to depict the forms, like a silhouette. Keep in mind, it's not the process that will be seen, but rather the final result. So imagine the result, and do whatever is necessary to convey that- skip any step you'd normally take if it isn't necessary or if it produces undesirable results.
