Sometimes you have to hard-refresh to make your updated image show up for you, because your browser caches it. So, it might've been getting updated fine.

Was the "Edit" button missing? I'd like to know so I can report that, because last I heard, that issue was apparently fixed.
It takes time to get approved, especially when you resubmit a bunch of times. The fastest way to have your work accepted is to submit it without any issues the first time.
