It's Wonderwice from Bleach.

I think in a chapter he was found with a dragon-fly and ate it or something....I don't remember...but it kind of explains the dragon-fly in the artwork.

I made the frog a hollow XD (lol)

The hole in his chest means that he is a hollow(a creature of the dead that eats human souls). He is in human form because his leader found a way to make the creatures to look like that. Hope that answers your question.
