Most of the extra colors were caused from when I moved the silhouettes around and it warped the colors. It is fixed now. I did use a reference and have posted the link below. Thank you, Naji, for the helpful link.

The top mushroom's shape is a lot better than the other two. Did you use any reference for this, if so, could you provide it please?

if you used layers make sure they're all on 100% opacity when you merged the final png file.
the mushroom shapes look great but I'm not too sure about the yellow outlines
You've got about 160 colors. Notice that there are a bunch of different shades of the 4 required colors and a couple of transparent pixels.
